Young Somerset is commissioned by NHS Somerset Integrated Care Board (ICB) to deliver a Community Wellbeing Service, where practitioners are trained to deliver interventions for children and young people aged 11-18
Young Somerset is also commissioned to work in partnership with Somerset NHS Foundation Trust to deliver Mental Health Support Teams (MHST) in schools.
The challenge
Young Somerset found they did not have a streamlined approach to collecting data, and often found themselves duplicating reports. To support their collaborative working model, they needed a system that could enable their practitioners, and those from Somerset NHS Foundation Trust, to access and securely share clinical records across different community locations.
The solution
Young Somerset chose iaptus as the digital care record system to support their NHS commissioned mental health work and their Somerset MHST. Furthermore, iaptus meets their needs by allowing them to manage records electronically and securely access the system from any location.
The system is designed by Mayden to support psychological therapy teams in CAMHS settings and other NHS commissioned services, with a care pathway at its centre that can be customised to different service configurations.
